What is Trochanteric Bursitis?

Trochanteric bursitis is an inflammation of the trochanteric bursa, a thin fluid-filled sac between the two gluteal tendons on top of the greater trochanter, the part of the femur felt at the side of the trouser pocket level. When this bursa becomes irritated, it causes pain over the side of the hip and is the most common cause of lateral hip pain.

This condition is also known as greater trochanteric pain syndrome (GTPS). It is prevalent and often straightforward to treat, though for some patients it can be more persistent. For a fuller breakdown of causes and symptoms, see our main trochanteric bursitis guide.

Why Does Trochanteric Bursitis Occur?

Many of the body’s major joints, including the elbow, shoulder and knee, contain bursa sacs. These fluid-filled pouches lubricate joints and tendons and help prevent friction between moving parts.

Our hips are among the largest and strongest weight-bearing joints in the body, supporting us as we walk and absorbing impact as we jump. This relies on balanced support from the surrounding muscles, tendons and ligaments, and any imbalance can lead to soft tissue or joint problems.

Similar to other bursae, the trochanteric bursa can become inflamed when the hip is overworked or injured. Trochanteric bursitis affects around five in every 1,000 people, and while it is more common in middle-aged or older adults, anyone can be affected.

Trochanteric Bursitis FAQs

What are some of the reasons for trochanteric bursitis?

Adults who walk, run, or cycle frequently may be susceptible to trochanteric bursitis. This is often due to tightening of the iliotibial band (ITB), a band of connective tissue running from the hip to the knee, which can irritate the trochanteric bursa if overused, resulting in bursitis or gluteal tendinopathy. Other possible causes include muscle injuries to the hip, a sudden increase in exercise intensity, post-total hip replacement, inflammatory conditions such as gout, and poor posture.

What are the Symptoms of Tronchanteric Bursitis?

The first sign of trochanteric bursitis is usually pain on the outside of the hip, which may start sharp before becoming a deep, dull ache. Walking, prolonged sitting, climbing stairs and lying on the affected side can all aggravate it. If left untreated, the pain may travel towards the knee, and the area becomes touch-sensitive. Consult a clinician if symptoms persist for more than two weeks.

How is Trochanteric Bursitis Diagnosed?

When you visit Joint Care Clinics, we carry out a physical examination and ultrasound scan to determine the source of the pain and suggest an appropriate treatment option, such as rest, hot and cold therapy, pain medication or an ultrasound-guided cortisone injection. After your consultation, our clinicians discuss these options with you, and if you prefer, you can be treated at the same appointment. Occasionally X-rays or, rarely, an MRI may be needed, and in some cases a diagnostic injection can confirm the source of the pain.

Is there anything I can do prior Trochanteric Bursitis Treatment?

Before considering injection therapy, simple non-invasive treatments are often a good first step. These include ice packs applied to the sore area for 15 to 20 minutes every four hours, anti-inflammatory medication such as paracetamol or ibuprofen (consult a clinician if in doubt), rest using walkers or crutches where needed, and physical rehabilitation with a skilled therapist to rebuild flexibility and strength. Where these do not help, a physician visit may be needed, and in rare cases where the bursa is irreparable, outpatient surgery to remove it.

Conditions similar to Trochanteric Bursitis?

Conditions similar to trochanteric bursitis include low back radiculopathy, hip joint problems, IT band syndrome, gluteal tears, bony injuries, piriformis syndrome and iliopsoas syndrome.

How can trochanteric bursitis be prevented?

Since overuse causes most cases, prevention is the most effective approach. This means avoiding or changing the problem-causing activity, correcting underlying issues such as leg length inequality or poor posture, and using a shoe insert where needed. When returning to exercise, start slowly, use moderate force with fewer repetitions, and stop if unusual pain develops. Strengthening the gluteal tendons and maintaining hip flexibility also help. If self-care alone is not enough, our clinicians also offer ultrasound-guided cortisone and hyaluronic acid injections to support your recovery.

How long does Greater Trochanteric Pain Syndrome take to improve?

There is no exact answer to this. As everyone is different and has different underlying symptoms. Also, body healing differs from one person to another.
Usually, Trochanteric Pain should settle in 2-3 months. But can take years too. Intermittent flare-ups are also possible.

What are the guidelines when doing physical activity?

Start slowly and progressively increase your activity level.
Use moderate force and few repetitions.
Stop if an unusual pain develops.

In what ways can I prevent Trochanteric bursitis?

• Avoid exercises that place repetitive stress on the hips.
• Strengthening of gluteal tendons.
• Lose weight if necessary.
• For leg length discrepancies, consider a shoe insert.
• Maintain flexibility and strength in the hip muscles.
• When necessary, use a walking cane or crutches for at least one week.

At Joint Care Clinics, we offer clinical consultation and ultrasound diagnostic scans to confirm the diagnosis. We offer ultrasound-guided cortisone injections ultrasound-guided platelet-rich plasma [PRP] and hyaluronic acid injections for Trochanteric bursitis.
